At STA, where your concerns deserve special care, we have designed our proprietary forms to begin an in-depth assessment. The forms can be emailed or faxed at your convenience to registration@specializedtherapy.com or to 201-342-4346 attention Intake Department. You may also bring your forms with you to the appointment. Your therapist will review your forms and then ask a series of questions designed to help formulate a recommendation and initial treatment plan. Discuss your treatment plan with your therapist.
